Directions for Administering
the SRI Questionnaires

Directions for the Instructor:

  1. Ask a student or staff member to distribute and return the questionnaires to the office of the department. In large classes, other students may assist, but only one should be responsible for overseeing the process.
  2. Please refrain from making comments, except to clarify procedures.
  3. Read the next section (Instructions to Students) to the class and leave the room. The instructor must not be present when the questionnaires are being filled out.

Instructions to Students:
(to be read to the class by the instructor)

There are two pages: a questionnaire and a sheet for your comments. They have the following instructions printed on them:

  1. The information you give will be used to review the effectiveness of your instructor's teaching. Your thoughtful ratings on the questions will be used in the faculty tenure and promotion decision-making process, for other personnel decisions, and to provide information on teaching effectiveness at Dalhousie University. Your response is anonymous. Your instructor, the Chairperson, and other relevant personnel committees will be provided with a summary of the class responses but will not see any of the completed questionnaires. Your response will not effect your grade. The summary of responses will be given to the instructor only after the final grades have been submitted.
  2. The instructions for filling out the questionnaire are in the upper right-hand corner. Use an HB or soft lead pencil. Do NOT use ink or ballpoint pens. Make heavy black marks that fill the circle completely. Erase clearly any answers you wish to change. Make no stray marks on the answer sheet.
  3. On the comments sheet, please feel free to comment on any aspect of the instruction. Please sign the form if you want your comments to be used for tenure, promotion, re-appointment, or other personnel decisions. All comments will be typed and any signatures deleted before being given to the instructor. If you choose not to sign, the typed version of your comments will be given only to your instructor.

Directions for Staff or Student Assistant:

  1. When the professor has left the room, distribute one questionnaire and one comment sheet to each student.
  2. Remind the students to use a pencil to fill out the questionnaires. Tell them it takes about 10 minutes to finish.
  3. Ask the students to work independently and refrain from discussing their responses. Collect all completed and any unused questionnaires and comment sheets. Put the forms back in the envelope. It is important that all forms be returned -- even the unused ones.
  4. In the presence of other students:
    1. seal the envelope,
    2. write the number of completed questionnaires on the label, and
    3. sign your name over the sealed flap.
  5. Return the envelope to the departmental office.
